This is a sole trader private speech pathology practice under the ownership of Dr Margaret Pozzebon. Dr Margaret Pozzebon is committed to providing and maintaining quality services, and respect and manage your privacy.
This Speech Pathology private practice complies with the Australian Privacy Principles (APPs) under the Privacy Act 1988, other privacy laws that govern how private sector health service providers handle your personal information, and Speech Pathology Australia’s Code of Ethics (2010).

This document outlines the ways we collect, use, disclose, store, secure and dispose of information is in line with legislation and standards.
What is personal Information and why do we collect it?
Personal information is information that identifies an individual. When this information is collected, we will explain why it is needed and how it will be used. I require this information to assist the speech pathologist to diagnose and treat you. If you provide incomplete or inaccurate information to us or withhold personal and health information, I may not be able to provide you with the services you are seeking.
How is this health information collected?
Usually, this health information is collected directly from you. Sometimes, I may need to collect information about you from a third party (such as a relative or another health service provider).
Disclosure of personal information
I will only disclose information to you, or people who are authorised by you. Disclosure of your information is shared with other health professionals involved in your care as nominated by you. An assessment, review or discharge report is usually sent to your referrer (such as, your GP, Geriatrician, Neurologist, NDIS) as required for your care. If you do not wish us to provide this information to your medical health provider, please let me know. On your request, I may also provide information about your condition to your partner, children, other relatives, close personal friends, or to a responsible person for your care.
Security of personal information
Your information is stored securely and protected from unauthorised access. Information will be kept for a minimum of seven years. When your information is no longer needed, we will take reasonable steps to securely destroy it.
Access to your personal information
You may access the information we hold about you. Please write to us if you wish to access your information. We will not charge a fee for your request but may charge a fee for providing a copy. To protect information, we will require identification.
Maintaining your personal information
It is an important to us that your information is up to date. I will take reasonable steps to make sure that your information is current and complete. Please let us know if your information changes so we can update our records and ensure we can continue to provide quality services.
